I know I'm supposed to be automatically prompted when I start the game but it never happens. Both my brother and I use this xbox and neither of us has been prompted to download the patch, despite the fact that the internet connection is without a doubt completely functional. I have PS3 so I dunno, but for the companion glitch, try going to Vault 22 and using the elevator. I dunno if its been confirmed for 360, but for PS3 its been found that when you use the elevator there is some kind of prompt for your companions. Many people have used this to get their companions back, as when you use the elevator they reappear with you if they are lost. So i dunno, maybe try that in the meantime, hope it works. When patching Fallout 3, can I just install the latest patch or do I need to install all previous patches?
Wherever you got the patch from it should say what else it requires. Some patches require you patch to a particular version before you install the latest patch, other patches are an all in one. If unsure keep all previous patches then install the latest patch.

They have already stated they will not be changing the ending so you can continue after the last quest, the game will always have a definitive ending.
Your only option is to start the game again. Also it's advisable to have a high level character before starting the Dead Money DLC so if you do start again you'll need to level up to around level 25-30.

yes i had beat fallout 3 3 times due to the trohy release and then the expansions. trophies dont read you already existing saved data so you have to start all over.
